Prostate Cancer nomogram are designed to help physicians and patients decide which treatment approaches will result in the greatest benefit. It is recommended that patients using the tool consult with a physician or other professional healthcare provider prior to making any treatment decisions.  There are a number of these nomograms that may give different answers - its your decision which to rely on.  following is some of the information you will need.  Different nomograms may want additional information or may not use all here.

Pre-Treatment

If you have not yet received treatment for your prostate cancer, you will need to have the following information in order to use the nomogram:


your most recent PSA (prostate specific antigen) value

your primary and secondary Gleason grade

your doctor's assessment of your clinical stage (using the 1992 or 1997 UICC system)

if you've already seen a radiation oncologist, the radiation therapy dose that he or she would deliver.

the number of positive cores found during biopsy

the number of negative cores found during biopsy

whether or not you have had neo-adjuvant hormones

whether or not you have had neo-adjuvant radiation


Post-Surgery

If you have already received surgery (prostatectomy), you may need to have or to know the following information to complete the nomogram:


your PSA (prostate specific antigen) value prior to surgery (and prior to hormone therapy, if you received it)

your PSA at the time of prostatectomy

your primary Gleason at surgery

your secondary Gleason at surgery

your prostatectomy Gleason Sum

the year of prostatectomy

the months disease free

whether or not your surgical margins were positive

whether or not cancer was found in your seminal vesicles

whether or not there was extra-capsular extension

whether or not cancer was found in your lymph nodes (if any were removed)

pre-radiotherapy PSA

radiation dose (if applicable)

whether surgical margins were positive or negative (if applicable)

whether there was seminal vesicle involvment (if applicable)

whether there was lymph node involvment (if applicable)

whether there was extra capsular involvement (if applicable)

whether or not you have had neo-adjuvant hormones

whether or not you have had neo-adjuvant radiation



Hormone Refractory Treatment

If you are considering receiving hormone refractory treatment, you will need to know the following information to complete the Hormone Refractory portion of the nomogram:


your Karnofsky Performance Status (KPS)

your Hemoglobin (HGB) value

your PSA

your Lactate Dehydrogenase (LDH) value

your Alkaline Phosphatase (ALK) value

your Albumin value

Additional Tools

The additional tools portion of the prostate nomogram -- which includes statistical devices measuring volume and PSA doubling time -- is not required to complete the nomogram. However, if you would like to use any of these features, you will need to have the following information:


Volume

To use and complete the volume portion of the nomogram, you will need to know the following information:

 

Prostate length

Width/transverse

Prostate height

Pre-treatment PSA

 

PSA Doubling Time

To use and complete the PSA doubling time portion of the nomogram, you will need to know the following information:

 

at least two PSA test results

the date the tests were taken
